As we age, we lose volume in certain areas of our face, and other areas descend due to increased elasticity and skin laxity. Dermal fillers (with hyaluronic acid, usually) are used to restore volume and help reposition descended tissue. However, dermal fillers are temporary, and can get quite expensive.
Facial fat transfer is an alternative where fat is harvested from the cheeks, neck, or body, prepared, and transferred into the face. This fat survives by direct contact with the tissue as little vessels grow into the fat. Not all fat survives however, with approximately 50% of the fat not surviving. This is why surgeons slightly over-inject fat transfer. Final results take 6 months to appear.
Fat transfer is usually a good solution for patients requiring larger volumes of facial injectables and who desire a more permanent solution.
Fat can also be further purified into smaller particles like sharp-needle intradermal fat (SNIF) and nanofat. SNIF uses fat as a permanent filler for wrinkle correction, Nanofat uses stem cells from the fat for skin regeneration

How Does Fat Injection Surgery Work?
With a fat transfer procedure, fat is harvested from the cheeks, neck, thighs or abdomen and is injected into the face to enhance facial volume and contours. The fat is harvested by liposuction and prepared using a special procedure to be reinjected into the face as a permanent and natural filler that we like to call “liquid gold”.
After injections, around 50% of the fat is lost. But that’s normal, with the rest of the fat staying permanently. Final results are at 6 months.
Fat grafting is an appealing option for certain patients, as a safe and durable alternative to dermal fillers. With fat grafting, surgical incisions are not required and it is minimally invasive.

The ideal candidate for the injected fat procedure is a patient with realistic expectations that wants a more permanent solution to facial volume loss, and that is not planning any extreme weight variations as this can affect results.
A minimal amount of fat must also be present in the cheeks, neck, abdomen or thighs. The procedure can be performed under local or general anesthesia depending on patient preference.

The facial fat transfer usually takes an hour and a half to perform. The final results take six months to show but surgical results last a lifetime.
Patients can return to work, school, or social activities the next day. However, swelling is to be expected especially during the first few weeks. Light physical activity can be resumed after a week, but you should refrain from heavy lifting for three weeks to prevent bleeding.
